Vision Loss Rehabilitation Alberta (VLRA) is a health services organization. We provide training that enables people who are blind or partially sighted to develop or restore key daily living skills, helping enhance their independence, safety and mobility. Our certified specialists work closely with ophthalmologists, optometrists and other health care professionals, providing essential care on a referral basis in homes and communities across Alberta. Vision Loss Rehabilitation Alberta is operated by CNIB and funded by donations and partially funded by the Government of Alberta.
- Patient will receive guidance to visit an optometrist/ophthalmologist for an updated eye report. In order to receive service from VLRA, a referral form from an optometrist, ophthalmologist or neurologist is required for each incoming patient. Note: A patient does not need to be legally blind to access services. Patients with any degree of vision loss should visit CNIB. Please be diligent in responding to the referral forms as quickly as possible as some patients may be in a very serious time-sensitive situation and require immediate assistance.
- When filling out the referral form, please make sure to indicate as much detail as possible on patient's vision acuity, diagnosis and field of vision. This information heips to make our intake process more efficient so that the patient can begin his/her vision rehabilitation as soon as possible.
- After the referral form has been received and the patient has come through our intake process, they now become a client of VLRA and are able to access the appropriate services and technology to help them better use their remaining vision.
- Referring physician will receive feedback in written form which outlines the services that their patient received at VLRA. This report is sent via fax to the general fax number of the optometrists, ophthalmologists or neurologist's office. Please don't hesitate to reach out to the **VLRA** Registrar to enquire about delivered services or any other information found in this report.
