To book the Classroom Activity Kit, fill out the Equipment Request Order Form. This kit includes the simple activity instructions, books to read to the class, and all of the equipment you need for the Eye Spy activity session. It will also include “thank you activity” cards for you to leave with students. Each of the cards will include your name and the name of your clinic, an activity for the child and information for parents on the reverse side.
